Street Health Center – Dorchester
Bowdoin Street Health Center (BSHC) has been
an integral part of the Dorchester community since 1973. BSHC provides
high quality, compassionate, and culturally competent primary and specialty
care services. BSHC is distinguished for its commitment to working in
partnership with community residents and businesses to successfully address
many community identified health needs of our neighborhood. The health
center is an economic anchor for the community, serving as the largest
employer. Bowdoin Street is licensed by Beth Israel Deaconess Medical
Center.
Dimock
Community Health Center – Roxbury
Dimock Community Health Center (DCHC) is the successor of the New England
Hospital for Women and Children founded in 1862 by women physicians. DCHC
is located on a beautiful nine-acre campus, which is listed on the National
Register of Historic Sites. DCHC provides integrated and comprehensive
health and human services through its three main program areas:
Health and Community Care: Primary care and related
services aimed at providing health maintenance, screenings, disease prevention
and management including, adult, pediatric and OB/GYN services; eye-care
and dentistry; HIV counseling, testing, outreach, case management and
education; women’s health services; free mammograms and menopauses
programs, specialty care in HIV medicine; infectious disease prevention
and treatment; diabetes diagnosis and control programs; podiatry; orthopedics;
tobacco treatment services; and childhood asthma prevention and treatment
programs.
Child and Family Development Services: Programs to
promote the healthy development of children and families including: Pre-school,
Early Headstart and Headstart development services; early intervention;
Teen Center, adolescent peer leadership and after-school programs; and
the Mary Eliza Mahoney Transitional Housing program.
Behavioral Health Services: Substance abuse, mental
health and development disability programs including: Intensive outreach
for the severely and persistently mentally ill; individual and group counseling;
psychiatry; child, family and school-based treatment; residential recovery
programs; and services for dually diagnosed women with HIV.
Fenway
Community Health – Boston/Fenway
Fenway Community Health is renowned nationally for its expertise in providing
care to the lesbian, gay, bisexual and transgender communities as well
as its sophisticated research programs. Fenway conducts scientific studies
in a variety of areas, and is recognized around the world for cutting
edge work on HIV prevention and transmission, including participation
in a number of national HIV vaccine trials. A member of Boston’s
Center for AIDS Research, Fenway Community Health also serves those who
live and work in Boston’s diverse Fenway neighborhood, including
many students, elders and people of color
Joseph
M. Smith Community Health Center – Allston/Brighton and Waltham
The Joseph M. Smith Community Health Center has been providing quality
primary health care services in the Allston and Brighton communities for
over 30 years. In September, 2004 the Health Center opened a second site
located in downtown Waltham. Joseph M. Smith CHC is the only federally-qualified
community health center in these communities.
The Health Center’s mission is to provide comprehensive,
culturally competent and affordable primary health care services and selected
specialties to all families and individuals in Allston/Brighton, Waltham
and surrounding communities. No matter what happens in our patients’
lives, Health Center staff will be there to assist them. Whether they
are children or adults, insured or uninsured, employed or unemployed,
long-term residents or recently arrived immigrants, the Health Center
will be there to provide the highest quality healthcare. We see ourselves
as the health care provider of choice for anyone who values diversity
in its many dimensions and inclusion of all groups with the community.
Outer
Cape Health Services – Orleans, Provincetown, Wellfleet
For nearly 40 years, Outer Cape Health Services, Inc. has been watching
over area residents and visitors with a comprehensive program of primary,
preventative, urgent and emergent health care services. It provides high
quality, cost-effective, culturally appropriate, accessible and affordable
health care for those who live in and visit the communities of lower and
outer Cape Cod including Provincetown, Truro, Wellfleet, Eastham, Orleans,
Chatham, Brewster and Harwich. Its three community health centers in Provincetown,
Wellfleet and Orleans are operated by a team of 12 dedicated medical providers
and allied health professionals who share a common mission of community
health care and well being.
Services include: comprehensive primary care; preventive, urgent and emergent
care; well baby care; women’s health; senior care; a confidential
teen clinic; HIV/AIDS testing, counseling, patient care management and
social services; radiology, laboratory, one of the state’s most
successful Women, Infant, Children programs with 5 WIC sites from Harwich
to Provincetown; and, the Healthy Connections program, which advocates
for patients by removing barriers to care and increasing access to primary
care at the community health level. In 2004, Outer Cape Health Services
received its second three-year accreditation from the Joint Commission
on Accreditation of Healthcare Organizations
Sidney
Borum Jr. Health Center - Boston/Theatre District & Boston Common
The Sidney Borum, Jr. Health Center offers a unique set of services
to adolescents and young people, including many gay, lesbian, bisexual,
and transgender young adults, street workers and runaway youth, and those
without health insurance. Its service delivery model is tailored to the
needs of its patients and community including street outreach and a new
mobile van service.
South
Cove Community Health Center -
Boston/Chinatown and Quincy
South Cove Community Health Center has been offering comprehensive bilingual/bicultural
primary and specialty care services to the Asian community for over 30
years. It has become a trusted resource for high-quality medical care
for long established and newly arrived immigrants. Most recently, South
Cove’s leadership in Asian health matters was recognized by the
Center for Disease Control, which called upon the health center to spearhead
a citywide effort in educating residents about SARS.
